Aidwell Healthcare is Hiring a Service Coordinator/Case Manager Near Long Island, NY

We are currently seeking a compassionate and professional service coordinator to join our growing team (Remote/Work from home).

We are a fast growing family run service coordination agency for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid Waiver Programs. We truly take pride in going above and beyond in helping people and keeping our team happy.

The NHTD and TBI Medicaid Waiver Programs are statewide programs that provide the support and services necessary in assisting seniors and individuals with a disability, or traumatic brain injury, to successfully live in the community and out of institutionalized settings. We advocate so seniors and individuals with a disability, or traumatic brain injury, have the same rights and opportunities as others.

Job Duties and Responsibilities

  • Act as the participant's advocate, provide unbiased assistance to the participant with the selection of providers, and where appropriate, assist the participant in locating a place to safely live in the community.
  • Development of Participant's service plan. This shall include, but not be limited to, completion of Individualized Service Plans and Plans of Protective Oversight (PPO); meeting with participants in order to gain an in-depth understanding of their goals; and meeting with significant individuals, as well as agencies, providing services in order to establish a comprehensive understanding of the participant's need for services.
  • Provide each participant a Waiver Contact List, as well as provide a review of this Contact List to assure it is correct, up-to-date and redistributed on an as-needed basis.
  • Provide assistance to waiver participants in terms of implementing the plan, as well as reviewing its effectiveness.
  • Coordination and scheduling of waiver and non-waiver services.
  • Maintain consistent and open communication with waiver participants.
  • Assure timely implementation of the approved service plan.
  • Conduct face-to-face meetings with the waiver participant at least monthly to monitor health and welfare needs as he/she adjusts to the waiver program.
  • Schedule team meetings.
  • Maintain case records consistent with all contacts and necessary documentation.
  • Maintain a tracking system that monitors all areas of compliance.
  • Practices proper safety procedures and take precautions at all times in order to ensure their safety, as well as the safety of their coworkers.
  • Reports all safety hazards or concerns to their Supervisor in a timely fashion.
  • Comply with all company policies and procedures.
  • Accepts any other responsibilities that management deems appropriate to the position.
  • Complete any incident reports as needed in timely manner (obtain provider 24-hour report if discovered by another provider or SC provider, 24-hour SC report, one week follow up, 30 day follow up, 60 day follow up, 90 day follow up and all subsequent monthly reports until incident is closed, investigative report and obtain any discharge papers from hospital), as well as present any incidents to the Incident Review Committee.
  • Assist individuals in scheduling medical appointments if needed, as well as providing linkages and referrals to new medical providers when needed.
  • Must be able to troubleshoot and navigate potential solutions with the team and communicate such matters with supervisor for resolution management.

Qualifications

  • Associate’s Degree with Five (5) years’ experience providing Service Coordination.
  • A Bachelor’s Degree and Three (3) Years’ experience providing Service Coordination.
  • A Master of Social Work, Master of Psychology, Registered Nurse, Certified Special Education Teacher, or Rehabilitation Counselor.
  • NYS Valid Driver’s license and a willingness to travel locally.

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
